In July 2015 he made his international debut as an assistant referee, he officiated Fiji vs. Tonga in the opening game of the 2015 World Rugby Pacific Nations Cup as an 2016 as an assistant referee for the games between assistant referee.
O’Keeffe made his Six Nations Championship debut in Italy and England and Ireland vs. Wales on February 7.
O’Keeffe made his international officiating debut on June 11, 2016, when he presided over the historic encounter between Samoa and Georgia, which resulted in a 19-19 tie. He officiated his first Tier 1 nation the following week, officiating Scotland’s opening test match against Japan on June 18. In August 2016, when South Africa played Argentina, he was selected as the assistant referee for his first Rugby Championship game.
